Terms and Conditions for Diagnostic Assessments
DBS Certificate Number 001988762174 October 2026
Payments terms:
1. Payment by BACS is required, and an invoice will be sent once the assessment has taken place. My current charge for assessments is £525. An optional Zoom feedback call with parents and the school is available at an additional cost of £25 for up to 30 minutes. A free feedback call with parents is also available upon request.
2. Payment of a £50 deposit must be made within 1 week of agreeing the date of assessment to secure the booking. The balance of £475 is due on the date of the assessment. Reports will not be issued until the balance is paid in full.
3. In-line with the Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013, there is a 14-day cooling off period where the assessment may be cancelled. The 14-day period begins whenever the deposit is received or the signed terms and conditions are received, whichever is the sooner.
Pre-assessment paperwork and conditions:
4. Where co-occurring needs (e.g. other diagnoses, anxiety) mean extra accommodations may be needed, this should be discussed with me before the assessment.
5. The person being assessed (student) must have an eyesight check by an optometrist (“optician”) within the last two years.
6. Visual difficulties questionnaire: Please complete the visual difficulties questionnaire with the student and return prior to the assessment
Visual difficulties (identified in the visual difficulties questionnaire) may be required to be discussed with an optometrist BEFORE the assessment. Glasses must be brought to the assessment if required.
The background information questionnaires must be fully completed and submitted at least two weeks before the assessment.
Two questionnaires are required:
1) Family questionnaire
2) School questionnaire
The assessment process:
7. A full assessment usually lasts between 2 -3 hours. A quiet, uninterrupted space must be provided for the assessment to take place in. To ensure the student's comfort, they will need to bring a drink with them. If the assessment is at school, it would be helpful to be given the times for break and lunch so that they can keep their usual schedule.
Conditions under which the assessment may be terminated:
8. I reserve the right to pause or postpone an assessment where an individual becomes unduly upset by the process or is visibly unwell.
9. If the student shows signs of visual difficulty during an assessment, I reserve the right to withhold an assessment report until the extent/impact of any visual difficulties has been determined.
10. I reserve the right not to accept a referral, terminate an assessment or not circulate a report if:
1) The referral appears to be outside of my area of expertise
2) The client fails to cooperate with the requirements of the assessment procedure in such a way that may compromise the validity of the assessment
3) The client fails to comply with the terms and conditions outlined in this document.
Cancellations:
11. Please provide as much notice as possible for cancellations or to reschedule. If you need to contact me on the assessment day, please call 07834 385656. As I typically book several months ahead, there is likely to be a delay in any rescheduled appointment.
Assessment report:
12. The results will be analysed, and a detailed report will be sent by email as a password-protected PDF within four weeks of the appointment, provided that payment has been made. The report will be written to adhere to the SpLD Assessment Standards Committee (SASC) guidelines.
Minor amendments to the background information may be requested up to 30 days after receiving a report. Once a report has been finalised (no later than 30 days after the report has been submitted), the assessment process concludes.
GDPR:
13. All data will be held in accordance with current GDPR legislation covering data protection. Please see the GDPR policy which can be found here.
Complaints:
14. Complaints – for any complaints please contact us as soon as possible using the details below and we will endeavour to resolve any difficulties as soon as possible.
